Dear Patient, 

This letter is official confirmation that you are registered onto a local authority commissioned programme, which consists of an initial telephone assessment and a series of weekly 90-minute sessions, delivered over a period of 1 2 weeks. There will be a final telephone review at 6 months.   

 It is important that you attend these appointments to ensure you are getting the best results from the programme. If you are required to take time off work to attend these appointments, please share this letter with your employer.  

Cancelling and rescheduling: 

Cancelling and rescheduling: If you are unable to attend any of the appointments, please contact us as soon as possible. You can contact us over the phone on 0330 236 9102. 

Non-attendance: 

If you do not attend two consecutive appointments without informing us of your absence, you will be discharged from the programme.
